Health Care Incentives Improvement Institute, Inc. (HCI3) aims to improve health care quality and value with evidence-based incentive programs and a fair and powerful model for payment reform. HCI3 is a non-profit organization, guided by a Board of Directors that includes physicians, employers, health plans, and others.

HCI3 implemented their programs in communities across the country, working collaboratively with clinicians, hospitals, employers, health plans and others, through one of the channel as Bridges to Excellence® (BTE). BTE programs recognize and reward clinicians who deliver superior patient care.
BTE Recognition is based on clinical data, which clinicians and practices voluntarily submit to an independent performance assessment organization (PAOs) for scoring. PAOs are one of the two types of organizations who take part in the performance assessment for BTE Recognitions. PAOs score the data, while Data Aggregators (BTE Approved Registries) collect the data from clinicians for scoring. BTE’s Recognition programs cover all major chronic conditions, plus preventive care, office systems—and a real Medical Home measurement scheme to promote comprehensive care delivery and strong relationships between patients and their care teams. Which includes, Asthama, COPD, Cardiac etc.
